The instructions below put all the instructions in the ProQuest form (you can skip pages 1, 2 and 3 of the form). Pages 4 and 5 of this form are required for all PhD students at Penn State. Fill in page 5 by entering the 4-digit code and your major category that you`ll find in pages 8 to 11 of the form. Return pages 4 and 5 to the Office of Theses and Theses, as well as a copy of your first page and summary. You do not have to provide ProQuest with a printed copy of the thesis. They may also limit distribution at the time of closing; Simply include a ProQuest/UMI non-distribution memo filled out with your publication contract form. To re-authorize ProQuest/UMI, to distribute copies of your thesis or doctoral thesis, you can send an email request to disspub@umi.com in order to make arrangements. You must enter your name, document title and ProQuest publication number with your request. As a publisher, ProQuest offers your thesis for sale and pays you royalties each year when your 10% is $10.00 or more. If you did not limit the dissemination of your thesis when you signed your author`s contract, we assume that you want to broadcast your work as widely as possible.

If you are no longer right to make your work widely available, ProQuest may limit the sale of your work according to your instructions. Please contact the ProQuest Author Relations team at disspub@il.proquest.com or (800) 521-0600, ext. 7020, if this is your wish, or if you have any other questions about Amazon.com or Google Scholar theses portals and theses. Information about ordering copies can be found via the Campus Copy Center at the Penn State Multimedia and Print Center (this process takes about three weeks). Page 4, please check the traditional post at the top of the page and yes under SELECT PUBLISHING OPTIONS. Sign at the end of the page (all restrictions you want to impose on your thesis will be applied on the eTD website; this form will not be sent to ProQuest until all restrictions have been lifted, usually two years after closing). The tax on traditional publishing on page 4 is included in the tax you pay to Penn State. If you decide to register the copyright (page 6), proQuest will receive an additional $55. Copyright registration is optional and you will still own the copyright, even if you don`t want to register it.
